The Centers for Medicare & Medicaid Services (CMS) Center for Medicare and Medicaid Innovation is hosting an event for potential applicants to the Direct Contracting Model.

During the webinar, the Direct Contracting Model team will provide a demonstration of the application portal and the Provider List Submission Tool (PLST). Following this session, attendees will have an opportunity to ask follow-up questions.
Please note that all attendees will be muted throughout the webinar. CMS will be reviewing and answering questions submitted by attendees through the Zoom platform’s Q&A function. As such, attendees are highly encouraged to join via the Zoom platform to ask questions during the event.
